Forms collect a contact's details in one step: a name, an email, a question, a payment. Surveys do the same thing across several pages, with questions that can change based on earlier answers. Both save every submission to Contacts in the hub, so you can follow up automatically. This article shows how to build one and test it.
Before you start
Decide what you need to capture. Every extra field lowers the number of people who finish, so keep it to the essentials.
Answers that are not standard contact details (for example "What is your biggest challenge?") need a custom field. Add one while building, or create it first under Settings > Custom Fields.
To take payments on a form, connect your payment provider under Payments first.
Create a form
Open the Forms builder from Funnels & Pages. Forms live under Funnels & Pages; switch to the Forms area at the top. For a survey, switch to Surveys instead.
Click + Create New Form (or the equivalent for surveys).
Choose a blank form, or pick a template from the Template Library. Templates include Application, Contact, Donation, Order, Registration, Request and Subscribe.
Give the form a name you will recognise later.
Add and arrange fields
The builder has three tabs on the right: Fields, Styles and Advanced.
In Fields, drag the fields you need onto the form. Standard fields include Full Name, Email, Phone and a message box. Field types include dropdowns, checkboxes, radio buttons, date pickers, numbers, monetary fields, file uploads and plain text.
To store an answer in a custom field, add a custom field from the panel, or create a new one on the spot.
Drag fields up or down to reorder them.
Mark the fields you must have as required. Email or phone should be required if you plan to follow up.
For a survey, split questions across pages so each page stays short.
Order-style templates include a Payment element so you can charge on the form itself.
Style the form
Open Styles to set layout, colours, backgrounds, spacing, input and button styles, and borders.
Or apply a prebuilt theme first and refine from there. Switching themes replaces any style changes you have made, so pick the theme before fine-tuning.
Set what happens after someone submits
Open Advanced.
Choose either a confirmation message that appears on the page, or a redirect to a thank-you page (for example a page in Funnels & Pages).
Turn on Email Notification if you want your team alerted for each submission. Set the subject, recipient email, reply-to email and sender name.
Turn on Auto Responder to send an automatic email to the person who submitted.
Surveys also have logic settings here, so you can jump to different questions based on an answer.
Save and test
Click Save.
Preview the form and submit a test entry with sample details.
Confirm you see the thank-you message or redirect you set.
Open the form's Submissions tab and check your test entry is there with every field filled in correctly.
After the form is live, the Analytics tab shows views, responses, average completion time and completion rate.
Put the form to work
Add the form to a page in Funnels & Pages, or embed it on an external website. See "Embedding a Form on a Funnel or External Site".
Use the form as a trigger in Automations so a submission can tag the contact, send a welcome email, or add them to Sales Pipelines.
Troubleshooting
Submissions are missing a field. The field is probably not mapped to a contact field or custom field. Open it in Fields and check the mapping.
No notification email. Confirm Email Notification is on and the recipient address is correct, then check spam.
